High-powered magnets, button batteries and small parts. The three that account for the serious injuries, what the US regulator changed in 2022, and the test that takes four seconds.

The toy hazards that actually send children to hospital

Most toy-safety advice is a long list with no ranking, which is a good way to worry a parent without protecting a child. The injury data is narrower than the lists suggest. A small number of hazards account for most of the serious harm, and they are worth knowing precisely.

This is a summary of what regulators and clinicians have published. It is not our advice, and it is not a substitute for medical guidance.

High-powered magnets

This is the one that changed recently, and it is the one most parents have never been warned about.

Small, very strong magnets — the loose spheres sold as desk toys, and the magnets inside cheap construction sets — are dangerous in a specific way. One swallowed magnet passes. Two or more, swallowed at different times, can find each other through the wall of the intestine and clamp together. The tissue caught between them loses its blood supply. The result can be perforation, sepsis and surgery, and the child often has no dramatic symptoms early on.

The US Consumer Product Safety Commission put a mandatory standard in place that took effect in October 2022. It applies to loose or separable magnets that fit inside the agency's small-parts cylinder, and it requires them to be either weak enough or large enough to be safe if swallowed — a flux index below 50 kG²mm² is the threshold it sets.

Two details are worth carrying:

  • Toys are largely outside that rule, because toys are already covered by the separate toy standard, ASTM F963, which restricts loose hazardous magnets in products for children. The 2022 rule was aimed at the products that were escaping through the gap — desk sets and novelties sold to adults but ending up in family homes.
  • The rule is national, not universal. Products bought through overseas marketplaces are a recurring problem precisely because they may not have been made to any of these standards.

The CPSC's own estimate is roughly 26,600 magnet-ingestion cases treated in US emergency departments between 2010 and 2021, and it has linked the hazard to several child deaths since 2005.

Button batteries

Different mechanism, similar seriousness. A lithium coin cell lodged in the oesophagus generates a current that burns through tissue — the damage can begin within a couple of hours, and it does not require the battery to leak.

The practical points are about access, not about avoiding batteries. Battery compartments on toys are meant to need a screwdriver to open. Compartments that pop open with a fingernail are the ones to look at, and they are usually found on remote controls, kitchen scales, greeting cards and novelty lights rather than on toys.

Guidance from poisons centres and paediatric bodies is consistent that a suspected button-battery ingestion is an emergency, not a wait-and-see. Do not wait for symptoms.

Small parts and choking

The oldest hazard and still the most common. The regulatory basis is a cylinder roughly the size of a young child's throat; anything that fits inside it is a small part and cannot be sold in a toy for under-threes.

The domestic version of the test is a toilet-roll tube. If a toy or a piece of one drops through, it is not for a child who still mouths things.

Two things this test does not cover. Items that swell when wet — some beads and some foam — can pass the cylinder dry and not pass a throat. And parts that are not small parts yet: a wheel that will come off, a seam that will split, an eye stitched rather than moulded on.

Things that swell

A newer entry, and one worth knowing because it defeats the standard test.

Superabsorbent polymer beads — sold as sensory toys, vase filler and craft supplies — can be a few millimetres across when dry and many times that after absorbing water. A bead that passes safely through a small-parts cylinder in the shop can expand inside a child. US regulators and paediatric hospitals have both issued warnings about ingestion cases requiring surgery.

The general principle is broader than beads: any test you run on a dry object is a test of that object dry.

Hand-me-downs and recalls

Second-hand toys are sensible and they are also where withdrawn products live on. A recall removes a product from shops; it does not empty the lofts of a country, and a toy passed between three families has usually outlived any notice its original owner received.

Two habits cover most of it. Check the make and model of anything battery-powered or magnetic against the recall list published by the relevant national regulator — the CPSC publishes one openly, as do equivalent bodies elsewhere. And treat missing packaging as missing information: without the box, the age marking, the battery-compartment warning and the manufacturer are all gone.

The hazard that is not in any standard

Older siblings. A house with a seven-year-old is a house with small parts in it, and no age labelling on the box addresses the problem of a shared floor. Where the toys land is the part standards cannot regulate.

What is worth checking, in order

  1. Loose strong magnets anywhere in the house, including on the fridge and in adult desk toys.
  2. Battery compartments that open without a tool.
  3. Anything that fits through a tube, at the height a crawling child can reach.
  4. Cords and loops longer than a hand's width on anything near a cot.

That is a short list, and it is short on purpose. It is where the serious injuries actually come from.

If you suspect a child has swallowed a magnet or a battery, the published guidance from poisons centres and children's hospitals is unanimous: seek medical help immediately rather than waiting to see what happens.
